If you are under a doctor's care, please get a medical release prior to making your appointment.

There are many services that use electrical current, please advise therapist of any medications, surgeries, physical therapy, pacemaker, metal implants, automatic medication implants or any other medical condition.

I understand that any services I have scheduled with Renew Wellness Center and/or Central Florida Massage Clinics may carry a risk of injury. I recognize that as a result of these services, unpredictable side effects may occur, including, but not limited to; redness and/or irritation of the skin, inflammation, feelings of being light headed, blood pressure changes, and allergic reactions. I understand that a medical evaluation is advisable before commencing any exercise. supplement or wellness service program. I will continue to keep staff informed of any medical problems or change in my physical condition which may affect services or appointments scheduled at Renew Wellness Center and/or Central Florida Massage Clinics. I acknowledge that neither Renew Wellness Center and/or Central Florida Massage Clinics, its owner, employees or independent contractors are engaged in diagnosing or treating medical diseases or conditions. I expressly assume all risks to me associated with any service offered by Renew Wellness Center and/or Central Florida Massage Clinics, and waive any claim which I might otherwise bring against Renew Wellness Center and/or Central Florida Massage Clinics, its officers, directors, shareholders, employees, or contractors as a result of injuries resulting from or relating to my participation in one or more services.

Renew Wellness Center and/or Central Florida Massage Clinics shall not be responsible or liable for any article lost, stolen, or damaged in or about Renew Wellness Center and/or Central Florida Massage Clinics.
Renew Wellness Center and Central Florida Massage Clinics will not tolerate any inappropriate acts. I understand that my session will be terminated due to any form of inappropriate behavior. We are committed to professionalism and expect the same from our clients. Thank you 

 To better serve you and all clients, all will agree, that time is our most valuable asset. I ask that appointments be guaranteed against late cancellation or rescheduling with a Good Faith Deposit. 

Good Faith Deposit Required: via  Zelle, Venmo or Amazon E-Gift Cards

Incall-$50 for NEW Clients at the Time of Booking.

Outcall-100% for NEW Clients at the Time of Booking due to multiple hours.

Tours-50% for ALL Clients at the Time of Booking.

Established Clients: No Deposit Required
** NO REFUNDS** If you are pre paid in full or deposit and you are a No Show or No Text WITHIN 6 HOURS of Appointment unfortunately NO REFUNDS**CREDIT** WILL be given on account with me IF 6 HOURS or MORE notice is given to reschedule**$50 Booking/Reschedule Fee will be deducted from what is paid and balance will go towards next visit. No Refunds Credit Only with me. **TOURS Same Policy Applies as Above. If I have to Cancel/Reschedule (which is very rare) You have the option of credit with me or a refund.Thank You
